No - Creeper Overhaul is a standard resource pack. Download the .zip here, place it in your resourcepacks folder (Options > Resource Packs > Open Pack Folder), and select it in-game. OptiFine or mods are optional, not required.
The upload targets Minecraft 1.21, and PacksMC's converter can generate builds for most other Java versions - pick the release you play in the download section and the file is prepared for you.
